Support at Home services

Aurelia Community Services provides practical, respectful support to help older people remain safe, connected and confident at home. Services are available across Wangaratta, Beechworth, Albury–Wodonga and surrounding North East Victorian communities, subject to assessed needs, location and availability.

Aurelia delivers local services in partnership with Trilogy Care.

Personal care & medication assistance

Respectful assistance shaped around each person’s routines, preferences and care plan.

Reablement & mobility

Support to practise everyday skills, remain active and follow an agreed occupational therapy or physiotherapy plan. Aurelia does not replace qualified allied health assessment or treatment.

Dementia-informed support & respite

Trained, dementia-informed support that considers communication, familiar routines, changing needs, expressions of distress and the wellbeing of family carers.

  • Calm, respectful support that considers possible triggers, distress and unmet needs
  • Observation and documentation of changes in communication, mood, behaviour and daily routines to help identify patterns

Community participation & practical support

Support to remain connected, manage everyday responsibilities and participate in community life.

Hoarding & squalor support

Respectful conversations and practical support focused on safety, wellbeing and realistic next steps. Support depends on assessed needs, location, availability and agreed funding arrangements, which are confirmed during the enquiry process.

Aurelia delivers your agreed local in-home support. Trilogy Care is the registered Support at Home provider and manages your funding and care plan. Clients can choose self-managed or fully coordinated arrangements, with care coordination available through the fully coordinated option. Services are subject to your assessed needs, agreed care plan and budget.

Private funding

For privately funded support, we can discuss available options, including arrangements through Trilogy Care. We’ll explain the proposed services and costs before you decide.

CHSP and Support at Home: understanding your options

Understanding your home care funding can help you choose your next step, whether you already receive support or are exploring aged care for the first time.

CHSP

Commonwealth Home Support Program

  • Entry-level assistance for older people needing a little help at home.
  • Approved services are delivered by government-subsidised CHSP providers.
  • Contributions depend on the service and provider.

Support at Home

  • Assessed ongoing support across different funding classifications, from lower to higher needs, plus short-term pathways such as restorative care and end-of-life support.
  • Assessed needs determine approved services and funding.
  • Contributions depend on the service and financial circumstances.

Both programs require an aged care assessment. Some services overlap; the main differences are the level of support and how care is funded and organised.

Your home care questions answered

Is CHSP ending in 2029?

CHSP grant funding has been extended to 30 June 2029. This is not a closure date or an automatic transition date. In August 2026 the Health and Aged Care Ministers said the Government’s view is that, in the long run, CHSP should remain a standalone program. Its longer-term design is still subject to consultation.

I receive CHSP. What if I need more help?

If everyday tasks are becoming harder or your care needs have increased, contact your current provider or My Aged Care to discuss a review. Depending on your assessment, Support at Home may be suitable.

How do I apply for government-funded home care?

Contact My Aged Care on 1800 200 422 or apply through its website. An aged care assessment determines eligibility and the support you may receive.

Is Support at Home the same as a Home Care Package?

Support at Home replaced the Home Care Packages Program on 1 November 2025. It provides ongoing and short-term assessed support at home.

Can I enquire before my funding is ready?

Yes. Contact Aurelia while exploring your options or waiting for funding. We can discuss our local services and availability. Starting funded services depends on your funding and service arrangements.

Can I enquire for a family member or client?

Yes. We welcome enquiries from family members, carers and health professionals. Please ensure you have permission before sharing someone else’s personal information.

Trilogy Care

Aurelia delivers the agreed local in-home support. Trilogy Care manages Support at Home funding and care planning, with care coordination available through its fully coordinated option.
